Te Whare Tautoko

Te Whare Tautoko Service provides community support care which involves the use of community-based supports to prevent the need for more restrictive levels of care and maximises wellness. The Service is a recovery-oriented community-based assessment, treatment and therapy service for mental health and addiction problems and providing holistic response to co-morbidities.

Support includes meal planning and preparation, shopping, cleaning, washing; and medication management, personal care which includes hygiene and dressing. education, support and advocacy are also provided.

The Service runs between 7am and 10pm every day including weekends and public holidays. Timings of the visits are based on the tangata motuhake needs.

Referral is from Te Whatu Ora (Health New Zealand) Mental Health & Addiction Clinical Team and Te Waka Whaiora Trust internal referral.
